New Patient Forms
We have invested in secure software to allow both new and current patients to update their pertinent medical, dental, and insurance information online in the comfort of their homes. No more hurrying to your appointment, only to be met with a list of necessary forms. Fill out your forms at our Patient Portal, when it is convenient for you! We will update our electronic records and even check your insurance benefits prior to your arrival so that we can help you understand your policy prior to any treatment.
With everything going digital, we understand your concern about the privacy and security of online forms and email correspondence with our office. We make sure we are compliant with OSHA and HIPAA regulations for how patient data is gathered, maintained, and stored. We never share your information with a third-party unless we have explicit written permission from you.
We strongly encourage our patients to take advantage of our secure online forms to help them make their visit to our office easy and stress-free. However, we understand that some patients still prefer paper forms. View our printable patient forms here.
During Your Appointment
During your first visit to Andover Family Dentistry, our gentle hygienist will take digital X-rays and intraoral pictures of your teeth, gums, and jaw. After your images are taken, we will complete a thorough, but gentle dental cleaning or periodontal (gum) cleaning depending on your needs. Your visit will be concluded with a comprehensive exam and oral cancer screening.
After reviewing our findings, we will address any questions you may have. We want you to leave every visit feeling fully-informed about your dental health and treatment options.
